error while adding dataset from mysql database

I have succesfully created a datasource with my database but while adding a dataset i have this error

My database on mysql is named etablissement. i wanted to list all entries of the table eleve

Environment knowage server 6.4.1; mysql 5.6.12; firefox
in Data Set by (140 points)
